If you already have an appointment for your VA disability claim we will need you to bring a few things with you to make the process easier and assure that we can get your report out to you as quickly as possible. These things include:

-DD-214 Form

-Driver's License or Identification Card

-A list of any medications you are currently taking

       (including dosage and frequency)

-Any previous mental health rating decisions

       (if you have been previously rated by the VA)

Reviews of complete claim files are available upon request. Please contact us in advance if you are interested in that service.
